This module defines and discusses common chronic diseases, and the best practices to care for chronically ill patients. You will learn to identify common and chronic diseases, prevention strategies, health care associated infections and patient-centered care principles for infected patients. Care problems specific to patients with multiple conditions and standards and regulations also are discussed.
